YOUR COMPREHENSIVE CHECKLIST
All the paperwork and forms needed for the ’22-23 school year!
A child will not be assigned to a classroom and cannot start attending school until all forms and paperwork have been submitted to SSDS. All forms for the ’22-23 school year are due June 20, 2022!

REQUIRED FOR STUDENTS
- Online form to be completed or updated for ALL students, every year.

- SSDS “Emergency Information” online form
- For returning students, use your username and password to update emergency information. Must be updated and signed for the ’22-23 school year.
- For new students, please create an account. This will allow you to save your progress, return to the form, and update it. Please complete a separate emergency information form for each child enrolled at SSDS.

- Medical forms to be downloaded and completed for all new students (this includes siblings) OR any students with a change in health or immunization status (Exception: if your child turns 2 after June 20th, please wait until their 2 year old check up to complete these forms).

- OCC 1215 : Child Health Assessment
- To be downloaded, completed by parent and doctor and signed by a doctor. Only requires updating if your child’s health status has changed
- Includes DHMH 4620 : Blood Lead Testing Certificate. Only requires updating if your child has received a second lead test since first submission

- MDH 896 : Maryland Department of Health Immunization Certificate
- Must be completed by physician, or accompanied by physician-signed printout of immunization record. Only requires updating if your child receives any new immunizations

- Medical condition or allergy forms to be downloaded and completed only if your child has a medical condition or allergy

- OCC 1216 : Medication Administration Authorization Form
- Must be submitted annually if your child has medication to be administered at school, including emergency medications
- Medication must be provided in original packaging, including over-the-counter medications.
- Allergy Action Plan – every year if applicable

- Seizure Medication Administration Authorization – every year if applicable

REQUIRED FOR PARENTS/ADULTS PARTICIPATING IN CLASSROOM
Up to three family members may be designated as participating adults.
- Parent medical form to be downloaded and completed by new participating adults (this includes new families and any other parents/adults who have never participated in an SSDS class before and want to this year)

- Medical reports remain valid for the duration of enrollment at SSDS. Only requires updating if health status has changed
- TB tests and immunization records are only required if the examining physician deems them necessary

- Fingerprinting
- Please sign up here for fingerprinting at SSDS or on your own! We will have two fingerprinting dates at SSDS — Thursday, May 26th and Saturday, June 18th.
- Forms to be completed at home and brought to your fingerprinting appointment
- OCC 1260 : Release of Information (ROI) will be notarized by SSDS for free at appointment

- Results remain valid for the duration of enrollment at SSDS. Does not require updating

- Child Abuse Prevention Training and New Parent Orientation
- Both the orientation & the training will be held virtually in the evening of Tuesday, August 30th. Zoom link will be sent closer to the event.

Are some of you thinking to yourself, “Okay, my child has already attended SSDS, so the office has their medical forms (and nothing has changed with their health or immunization history), doesn’t have a medical condition or allergy that requires medicine at school, AND no one new in our family wants to participate this year…so am I all set besides making sure to update the Emergency Information Form above?” Yes, you are good to go!
After reading that statement are some of you now thinking, “No, none of that applies to me and I’m feeling slightly overwhelmed by all of this!” And maybe slightly is an understatement.
